This form is for children who are eligible to receive First Sacraments (First Reconciliation, First Eucharist) this year. 
To be eligible for this program,
1) The child's family needs to be registered parishioners at St. Stephen the Martyr, Renton, and 
2) The child must have taken one of the following classes during the 2019-20 school year:
     -1st Grade Faith Formation (Wednesdays @ St. Stephens)
     -Caves Class Faith Formation (Wednesdays @ St. Stephens)
     -Attended a Catholic School
(If you are not yet eligible to receive First Sacraments, go back to the website and register with a different form:  K-5th Grade Faith Formation.)


Complete one registration form per child.  For children not receiving First Eucharist, you need a different form. Find their form on our website. (K-5th Grade Faith Formation)

Medical Release
In case of emergency while my child is attending programs at St. Stephen the Martyr Parish, the parish staff has my permission to seek and authorize appropriate emergency medical treatment for my child(ren). I understand all efforts will be made to contact me as soon as possible. I agree on behalf of myself, my child(ren) named herein, or our heirs, successors and assigns, to hold harmless and defend St. Stephen the Martyr Parish, its officers, directors and agents, and the Corporation of the Catholic Archbishop of Seattle, chaperones, or representatives associated with the event, from any and all actions, claims, demands, damages, costs, expenses and all consequential damage arising from or in connection with my child attending the event or in connection with any illness or injury or cost of medical treatment in connection therewith, and I agree to compensate the organization, its officers, directors and agents, and the Corporation of Catholic Archbishop of Seattle, chaperones, or representatives associated with the event for reasonable attorney’s fees and expenses.

Photograph and Video Consent
At St. Stephen the Martyr, we like to use photos and videos obtained during programs for presentations, slideshows, flyers & publications, and the website. This connects us as a community. We need parent/guardian consent to do this. We will not post your child/teen’s name unless you give us specific permission, and then we will only use their first name. If you ever have a concern about images posted on the website, please contact the Faith Formation Office and we will remove them from the website.


I/We, the parent(s)/guardian(s) of the children/youth listed at the top of this page, authorize and give full consent, without limitation or reservation, to St. Stephen the Martyr Parish to publish any photograph or video in which the above named student(s) appear while participating in our faith formation programs and activities. There will be no compensation for use of any photograph or video at the time of publication or in the future.

Online Consent and Code of Content-  Part of our program will use online materials from our publisher. We will also use video conferencing, such as Zoom. With large-group gatherings, we'll need parents to attend with their children, monitoring their safety and experience. For the most part, these will be less than 30 minutes long.  If at some point we use breakout rooms, we will meet Archdiocesan safety criteria by monitoring all sessions and having two Safe Environment Certified Adults present during all session/online interactions between children and parish staff and volunteers. In this scenario, parents would not be required to attend smaller breakout classes

Code of Conduct for K-5 Programming (Online)-  To assure the safe and successful participation of children and adults participating in parish online programming in the Archdiocese of Seattle, the following code of conduct is to be followed. Violation of these norms may result in dismissal/denied access for future online sessions. All participants are expected to:


   - Follow Leader instructions;
   - Dress appropriately;
   - Show their real name;
   - Keep the video turned on; and
   - Show good classroom and community behavior.
