Medical Release
In case of emergency while my child is attending programs at St. Stephen the Martyr Parish, the parish staff has my permission to seek and authorize appropriate emergency medical treatment for my child(ren). I understand all efforts will be made to contact me as soon as possible. I agree on behalf of myself, my child(ren) named herein, or our heirs, successors and assigns, to hold harmless and defend St. Stephen the Martyr Parish, its officers, directors and agents, and the Corporation of the Catholic Archbishop of Seattle, chaperones, or representatives associated with the event, from any and all actions, claims, demands, damages, costs, expenses and all consequential damage arising from or in connection with my child attending the event or in connection with any illness or injury or cost of medical treatment in connection therewith, and I agree to compensate the organization, its officers, directors and agents, and the Corporation of Catholic Archbishop of Seattle, chaperones, or representatives associated with the event for reasonable attorney’s fees and expenses.

Photograph and Video Consent
At St. Stephen the Martyr, we like to use photos and videos obtained during programs for presentations, slideshows, flyers & publications, and the website. This connects us as a community. We need parent/guardian consent to do this. We will not post your child/teen’s name unless you give us specific permission, and then we will only use their first name. If you ever have a concern about images posted on the website, please contact the Faith Formation Office and we will remove them from the website.


I/We, the parent(s)/guardian(s) of the children/youth listed at the top of this page, authorize and give full consent, without limitation or reservation, to St. Stephen the Martyr Parish to publish any photograph or video in which the above named student(s) appear while participating in our faith formation programs and activities. There will be no compensation for use of any photograph or video at the time of publication or in the future.

Code of Conduct for Youth at Online Programs


Youth Ministry programming for our parish will be moving to an online video/audio platform while safety and social distancing orders are in effect for our archdiocese. We will be using ZOOM video conferencing services and ensuring we meet all safety criteria by monitoring all sessions and having two Safe Environment Certified Adults present during all session/online interactions between youth and parish staff and volunteers.

To have your teen participate, please review the Youth Code of Conduct for online program participation and acknowledge your consent below.

Youth Code of Conduct (online programming) – In order to assure the safe and successful participation of youth and adults participating in parish online programming in the Archdiocese of Seattle, the following code of conduct is to be followed. Violation of these norms may result in dismissal/denied access for future online sessions. All participants are expected to:


1. Follow Youth Minister or Adult Leader instructions at all times.


2. Dress appropriately.


3. All participants must login under their real name.


4. All participants must attend with their video on so that the participant is visible at all times unless instructed to turn off their video by the Youth Minister or Adult Leader.


5. Refrain from behavior or using language that is inappropriate. This includes but is not limited to:


a. bad language;


b. presentation of inappropriate images or music;


c. visible use or possession of alcohol, illegal drugs, or weapons;


d. disruptive actions that are tantamount to bullying, harassment, or verbal abuse.

Parental Permission
In marking "Yes" below, I, the parent/guardian of this teen, grant permission for my teen to participate in St. Stephen the Martyr online youth ministry programs under the direction and guidance of the youth minister/program volunteers.

If I mark "No" below, I am not granting permission for my teen to participate in online youth ministry programs, and they will not be attending online.
